ENT ST3 2027 recruitment includes several important changes for applicants. This guide covers the application timeline, eligibility requirements, portfolio and self-assessment preparation, interview planning and the key information you need for the 2027 recruitment cycle.
The national Otolaryngology ST3 2027 person specification and Round 2 recruitment dates are now available. However, the detailed ENT-specific 2027 self-assessment scoring guidance and interview format have not yet been confirmed in the published national material. Previous-cycle scoring is therefore shown below only as a preparation reference and should not be treated as the final 2027 scoring system.

ENT ST3 2027 Eligibility
Before working on portfolio scores or interview preparation, confirm that you meet the nationally agreed Otolaryngology ST3 2027 person specification.
Core Requirements
- Hold an MBBS, BMBS or equivalent medical qualification.
- Be eligible to work in the UK.
- Hold full GMC registration at the time of application.
- Hold a current licence to practise by the date the post commences.
- Provide a complete employment history, including relevant gaps.
- Demonstrate achievement of the required core surgical competences.
- Provide validated logbook evidence of surgical exposure to date.
- Meet the required professional examination criteria.
Core Surgical Competence
For 2027 recruitment, core surgical competence can be demonstrated through one of three recognised routes:
- Current employment in a UK Core Surgical Training programme leading to successful completion by the advertised post start date.
- Previous successful completion of UK Core Surgical Training, supported by a satisfactory ARCP outcome submitted with the application.
- A completed Alternative Certificate of Eligibility to Enter Higher Surgical Training, submitted with the application.
Applicants must successfully complete MRCS or MRCS (ENT) by the national initial offers released by date for the recruitment round.
For Round 2 2027, the national initial-offers deadline is 13 April 2027 at 17:00. Applicants planning examination sittings should therefore consider this deadline carefully.
New for 2027: Specialty Recruitment Prioritisation
The Medical Training (Prioritisation) Act 2026 applies to recruitment using the 2027 ENT ST3 person specification.
For 2027 recruitment, information used to determine an applicant's prioritisation status is collected through the application form. The resulting status is displayed in Oriel.
Applicants who believe their prioritisation status has been determined incorrectly can request a review before shortlisting begins.
For applications closing on 10 December 2026, the published deadline for requesting a review of prioritisation status is 15 December 2026 at 17:00 UK time.
ENT ST3 2027 Application Process
Applications are submitted through Oriel. Build your application around the current person specification and the final ENT-specific applicant guidance once released.
Review the 2027 person specification, including GMC registration, core surgical competence and MRCS requirements.
Provide a complete employment and training history and answer all eligibility and declaration questions truthfully.
Once the final scoring framework is published, claim only achievements that meet the precise wording of each criterion.
Make each claim easy to verify. Evidence should demonstrate exactly what you achieved, your role and the relevant dates.
Do not leave eligibility documents or portfolio evidence until the deadline. Follow the published instructions for document format, naming and upload.
ENT ST3 2027 Self-Assessment & Portfolio
The detailed ENT ST3 2027 self-assessment scoring framework has not yet been confirmed in the official material reviewed for this update. Do not assume that the 2026 scores, thresholds or evidence rules will remain unchanged.
The previous recruitment cycle assessed applicants across several portfolio areas. These remain useful areas to organise while waiting for the definitive 2027 guidance.

Participation and leadership within recognised research collaborations.
Evidence of meaningful participation, leadership and completed improvement cycles.

What Did the 2026 ENT ST3 Portfolio Assess?
Until the 2027 scoring document is released, the previous framework can help applicants identify areas of their portfolio that may require attention. However, the following figures are historical 2026 criteria, not confirmed 2027 scores.
ENT Experience
The 2026 framework awarded up to 20 points according to the amount of postgraduate ENT experience accumulated after Foundation training.
Postgraduate Qualifications
The previous framework awarded up to 5 points for eligible postgraduate academic qualifications, with higher scores available for research degrees.
Publications
The 2026 framework awarded up to 8 points across an applicant's two highest-scoring eligible publications, with scoring influenced by authorship, publication type and journal criteria.
Collaborative Research
Up to 2 points were available in the previous cycle, with higher recognition for significant leadership roles within recognised collaborative research.
Audit & Quality Improvement
The 2026 framework awarded up to 4 points across an applicant's strongest eligible projects, with emphasis on meaningful involvement, completed cycles and leadership.
Presentations
Up to 6 points were available across two eligible presentations in 2026, with scoring influenced by presentation type, authorship and meeting level.
Surgical Logbook
The 2026 framework awarded up to 25 points for validated operative experience across five broad areas:
- Ear procedures
- Nasal procedures
- Tonsillectomy
- Throat and neck procedures
- Emergency ENT procedures
Do not use the old procedure thresholds to predict a 2027 score until the new guidance confirms whether they have been retained.
How to Prepare Your ENT ST3 Evidence
Strong evidence should allow an assessor to understand your achievement quickly without having to infer what a document means.
- Keep your ISCP/eLogbook accurate, current and appropriately validated.
- Retain formal evidence of publications, presentations, prizes and postgraduate qualifications.
- Keep signed documentation of audit and quality-improvement activity, including your personal contribution.
- Document teaching, leadership and research activity contemporaneously rather than reconstructing it shortly before applications.
- Use clear filenames and organise evidence according to the final 2027 domains once published.
- Ensure submitted material contains no patient-identifiable information.
Never claim a higher category unless your evidence genuinely satisfies its requirements. If the evidence does not substantiate the claim, assessors may not award the requested score. Concerns about deliberately inaccurate or falsified evidence can have wider professional consequences.

ENT ST3 Interview 2027
Do not assume that the previous interview format will remain unchanged. The final 2027 specialty-specific applicant guidance should be used once the national recruitment team publishes the interview structure.
The 2027 person specification nevertheless gives a useful indication of the capabilities candidates should prepare to demonstrate.

- Revise clinical governance, Duty of Candour, consent and patient-safety principles.
- Prepare examples demonstrating leadership, teamwork, teaching, research, audit and quality improvement.
- Be able to explain your motivation for ENT and demonstrate a realistic understanding of the training pathway.
- Practise aloud under timed conditions rather than relying exclusively on written revision.
ENT ST3 Competition
ENT remains a competitive higher surgical training specialty. The previous version of this guide recorded 243 applications for 41 posts, giving a competition ratio of 5.93.
These figures relate to a previous recruitment cycle and should not be presented as the 2027 competition ratio. The 2027 competition ratio cannot be known until the recruitment cycle has taken place and official application and post numbers are available.
ENT ST3 2027 Offers
Offers are managed through Oriel. The national Round 2 timetable states that initial offers must be released by 13 April 2027 at 17:00.
The hold deadline is 21 April 2027 at 13:00, followed by the upgrade deadline on 22 April 2027 at 16:00.
Applicants should review the national recruitment guidance carefully when preferencing posts and deciding whether to accept, hold or opt into upgrades.
